First Time with Dermal Fillers: What to Expect and How to Prepare

Getting dermal fillers for the first time is a big step, and it’s totally normal to have questions, hesitations, and even a little nervousness. Maybe you’ve seen subtle, beautiful results on friends or scrolled past a few before-and-afters online and wanted that refreshed look for yourself. Maybe you have seen before-and-after photos of unrecognizable celebrities and don’t want that to happen to you.

Today’s dermal fillers are more advanced, customizable, and natural-looking than ever before. At Cascade Eye & Skin Centers, we specialize in helping patients feel confident and empowered in their decisions about cosmetic treatment. Whether you’re interested in softening lines, restoring lost volume, or enhancing your natural features, this guide will walk you through everything you need to know about getting fillers for the first time.

First Time with Dermal Fillers: What to Expect and How to Prepare

What Are Dermal Fillers?

Dermal fillers are injectable treatments designed to add volume, smooth wrinkles, and enhance facial contours. Most fillers are made with hyaluronic acid (HA), a substance your body naturally produces to maintain skin hydration and elasticity. As we age, our HA levels drop, leading to volume loss and fine lines.

Fillers help replenish what time and gravity have gradually taken away.

Popular treatment areas include:

  • Nasolabial folds (smile lines)
  • Marionette lines (lines from the corners of the mouth)
  • Cheeks
  • Lips
  • Jawline and chin

Each type of filler is designed with a different texture, density, and intended use, which allows your provider to customize your treatment to meet your goals.

Your Aesthetic Consultation: What Happens First

The first step isn’t a needle; it’s a conversation.

During your initial aesthetic consultation, your provider will:

  • Ask about your goals and areas of concern
  • Review your medical history and any medications
  • Examine your facial structure and natural expressions
  • Discuss treatment options and product recommendations
  • Talk through expected outcomes, risks, and pricing

At Cascade Eye & Skin Centers, we never take a one-size-fits-all approach. We’ll help you understand exactly what’s possible based on your anatomy, your preferences, and the science behind different filler formulations.

How to Prepare for Your Dermal Filler Appointment

While dermal fillers are non-surgical and minimally invasive, preparing your body for treatment can help reduce the risk of bruising, swelling, or unexpected downtime. Here’s how to set yourself up for a smooth experience:

One Week Before

  • Avoid blood-thinning medications and supplements, including aspirin, ibuprofen, vitamin E, fish oil, and ginkgo biloba (unless medically necessary and discussed with your provider).
  • Hold off on alcohol and excessive sun exposure, which can increase the risk of bruising.

Two Days Before

  • Let us know if you’re prone to cold sores, especially if you’re getting lip filler, as you may need a preventive antiviral prescription.
  • Start taking Arnica (available at Cascade) now or on the day of the procedure to help decrease bruising.

Day Of

  • Come with a clean, makeup-free face
  • Eat a light meal and stay hydrated
  • Plan for a relatively calm day after your appointment (not photoshoots or big events)

What to Expect During the Treatment

Your actual filler appointment will likely take about 30 minutes, depending on how many areas you’re treating.

Here’s what it typically involves:

  • Your provider will cleanse your skin. If treating the lips, numbing cream can be applied. Fillers also have lidocaine to reduce discomfort, and ice is also available.
  • Using a very fine needle or cannula, the filler will be injected into specific areas.
  • Your provider may gently massage or shape the filler to ensure a natural contour.
  • You might feel a small pinch or pressure, but most patients say it’s very tolerable and much quicker than they expected.

Aftercare: What’s Normal and What’s Not

Right after your treatment, you’ll likely notice mild swelling, redness, or tenderness at the injection sites. This is completely normal and typically resolves within a few days.

Things to avoid for 24–48 hours:

  • Touching or massaging the treated area
  • Intense workouts or hot environments (like saunas or hot yoga)
  • Alcohol and salty foods
  • Sleeping on your face

You may notice:

  • Swelling (especially in the lips) for the first 24–72 hours
  • Mild bruising; using Arnica or a cold compress can help
  • Most patients see noticeable improvement right away, with full results settling in after about 7–14 days.

What’s not normal:

  • Intense pain, white areas or mottled red/blue areas. This can be a sign of vascular occlusion, and you should alert your provider immediately.
  • Occasionally, large bruises can occur. If there are any concerns, contact your provider for an evaluation.

How Long Do Fillers Last?

It depends on the type of filler and where it’s placed. Generally:

  • Lip fillers last 6–12 months
  • Cheek or mid-face fillers can last 12–18 months
  • Deep fold or jawline fillers can last up to 2 years

Because fillers gradually dissolve, maintenance appointments can help you preserve your results without starting from scratch each time.

Dermal Filler Frequently Asked Questions

How soon will I see results after my filler appointment?

You’ll see immediate improvement in volume or contour after your treatment. However, mild swelling or bruising can temporarily mask the final result. Most patients see their true outcome after 14 days, once everything has settled.

How long does a filler appointment usually take?

Most dermal filler appointments take about 30 to 60 minutes, depending on the number of areas being treated. Your consultation, numbing time (if needed), and post-treatment instructions are all part of that window.

Will I need to take time off work or social activities?

Not usually. Fillers are sometimes called a “lunchtime procedure” because you can return to most daily activities right after. That said, you might want to plan for a day or two of downtime if you’re concerned about mild swelling or bruising. A mask can be worn, and we do have cover-up makeup that can be applied post-treatment.

What’s the difference between fillers and wrinkle relaxers?

Great question! Fillers add volume to smooth out lines, plump areas like the lips, or enhance contours like the cheeks or jawline. Wrinkle relaxers like Botox, Dysport and Daxxify relax muscles to soften dynamic wrinkles caused by movement (like frown lines or crow’s feet).

Will I look “overdone”?

Not with us. Our providers are experts in delivering natural-looking results that never leave you looking overfilled or unrecognizable.

How much filler will I need?

Everyone’s facial anatomy is different. Some patients only need 1 syringe, while others benefit from multiple syringes placed strategically. Your provider will walk you through what’s appropriate.

Is there a way to reverse it if I don’t like it?

We think you will love your results, but if dermal fillers aren’t for you, there is an option. Hyaluronic acid fillers can be dissolved using an enzyme called hyaluronidase. It’s rare that patients want reversal, but it’s an option for peace of mind.
